This is a technical term in nursing-ese; it's a 'nursing diagnosis' and it requires a 'nursing intervention' - there's a whole bunch of them and as a recent nsg student I can imagine what the rest of the index cards said. Did you find any cards reading Disturbed Sleep Pattern or Risk for Spiritual Distress??? Or perhaps Impaired Swallowing or Ineffective Breathing Pattern?? Same project...mystery solved.

It was a Wednesday Mystery of the Week, and it was from around the same time as Rock-a-Die Baby. C-c-creeeeepy!
...recognized this right away! It's a nursing diagnosis. It's really an exercise in futility for a nursing student, but still a requirement, so you have to know what this means.
Obviously this was someone's study card or cheat sheet for a test.
I've been a nurse for many years now and...Still hate care plans!
